During my appointment on June 3, 2025, the provider failed to document my reported symptoms and requested diagnostics. In accordance with my rights under HIPAA, I submitted a formal addendum to correct these omissions. During my follow‑up visit on January 19, 2026, the provider was dismissive of this addendum. When I requested confirmation that it had been properly integrated into my record, she refused to verify it and became evasive, declining to provide any clinical confirmation. Across both appointments, the provider displayed unprofessional non‑verbal behavior, including making disparaging facial expressions while I described my symptoms.
