Taylor Freedline, NP

Taylor Freedline, FNP-BC, a board-certified Family Nurse Practitioner, joined Primary Care Partners of South Bend in 2026. She provides comprehensive primary care for children and adults, bringing a strong background in nursing and a commitment to serving individuals and families in our community.

Taylor earned her Master of Science in Nursing from Indiana University South Bend, graduating with a 4.0 GPA. She also holds a Bachelor of Science in Nursing from Bethel University.

Taylor completed 300 hours of clinical training at Primary Care Partners before completing a 12-month Family Nurse Practitioner Residency at HealthLinc in Mishawaka. During her residency, she provided primary care to a diverse and medically underserved patient population, further developing her experience in caring for patients across the lifespan.

Before becoming a nurse practitioner, Taylor spent five years as a registered nurse. Her experience includes caring for patients in the neonatal intensive care unit and emergency department at Memorial Hospital in South Bend, as well as the intensive care unit at Elkhart General Hospital.

Taylor is board certified through the American Nurses Credentialing Center and is a member of the Coalition of Advanced Practice Registered Nurses of Indiana. Her clinical interests include preventive care, chronic disease prevention and management, lifestyle change, and pediatrics.

A longtime member of the Michiana community, Taylor was a four-year member of the Bethel University women’s tennis team. She served as team captain for three years, earned three All-American honors, and was named the 2019 NCCAA North Central Regional Player of the Year.
