Rachel Self, MSN, APRN, FNP-C

Rachel Self, MSN, APRN, FNP-C is an experienced provider with more than 10 years of clinical practice, offering compassionate, comprehensive care to patients of all ages, including women’s healthcare. She earned her Associate Degree in Nursing from Northwestern State University, her Bachelor of Science in Nursing from the University of Louisiana at Lafayette, and her Master of Science in Nursing from Herzing University.

Raised in a military family, Rachel had the opportunity to live in various parts of the United States and Germany throughout her childhood. This unique experience instilled in her a deep appreciation for diverse communities and cultures, shaping both her worldview and her approach to healthcare.

Rachel spent much of her nursing career in northwest Louisiana, where she developed lasting professional relationships. With deep family ties in East Texas, she and her family relocated to the Lufkin area and are proud to call the community home.

Outside of work, Rachel enjoys spending time with family and friends and has a passion for traveling.
