Jake is an excellent provider! He takes time to address my questions or concerns and always thoroughly explains any new procedures available that I may benefit from.

I asked for a handicapped placard for my pain as doing simple things like going to the grocery store can cause a flare up for several days for me. I have to take many sitting breaks when i am in public places where I have to walk. This provider told me that I did not meet the requirements. I asked what the requirements were and he listed them off and stated that “anyone who has a hard time walking 200 feet with cardiac issues”. I stated that I thought the DVS website says that 200 feet includes pain as well. He said “yeah I guess you’re right I can give you a temporary one for 6 months.”
I explained that it’s not because I am a young, lazy person that doesn’t want to walk far. I’ve lost 80lbs in the last year and my pain has gotten worse.
I felt like he was trying to lie to me by saying I don’t meet the criteria, when according to the DVS website, I do.
