HealthSource Lebanon Family Practice is located on Columbus Avenue, in the shopping center near the new Tractor Supply store. We are accepting new patients of all ages, accept most insurance plans—including Medicaid—and offer a discounted sliding fee program for uninsured patients.

Our family practice clinicians provide ongoing primary care for individuals and families, including sick visits, minor injury care, annual checkups, preventive visits, prescriptions, and chronic condition management for issues like diabetes and high cholesterol. To support total wellness, HealthSource Lebanon features integrated behavioral health services, giving patients access to mental and emotional health support right during their regular medical visits. We also offer Osteopathic Manipulative Treatment (OMT), a hands-on technique performed by trained clinicians to help relieve back, neck, muscle, or joint pain and improve movement.
