HealthSource Goshen School-Based Health Center and Family Practice

HealthSource Goshen School-Based Health Center and Family Practice is located at the rear of Marr/Cook Elementary School in Goshen, at 6696 Goshen Road. The center is open to the entire community and also serves the Goshen Local School District as a School-Based Health Center. HealthSource Goshen is open year-round, including during school breaks. We are accepting new patients of all ages, accept most insurance plans (including Medicaid, Medicare, and private insurance), and offer a discounted sliding fee program for uninsured patients.

Our family practice clinician provides comprehensive, ongoing care for patients of all ages—from newborns to seniors. Services include sick visits, minor injury care, annual checkups, preventive care, prescriptions, and chronic condition management for issues like diabetes and high cholesterol. By offering school-based health services directly on campus, we make it even easier for Goshen Local School District students and families to access convenient, high-quality care close to school.
