The visit went ok. After arriving home and reviewing the medical records in my portal, I noticed that she had put information that was not discussed in my medical records. It was stated that we had a “several minutes discussion on smoking cessation aids”. This conversation never happened nor am I a smoker. It is not acceptable to lie on patient records.
