Dr Kim gave clear instructions for upcoming surgery. Suggestion: I requested Handicap parking placard. Dr. Kim agreed, but form was filled out incorrectly, his Physician license #, a required entry, was missing. DMV had to call or lookup info, which took extra time and effort. (Perhaps this was an administrative error of his staff..someone should be aware of this). Also, this placard should be automatically offered to patient, shouldn't have to ask for it.
