David Dayya, D.O., is a triple board-certified physician whose career has been defined by a singular philosophy: “marrying science with service.” With advanced training in Primary Care/Family Medicine, Public Health, and Undersea & Hyperbaric Medicine, he brings a uniquely comprehensive and data-driven approach to patient care. In addition to earning his medical doctoral degree from the University of Medicine and Dentistry of New Jersey, Dr. Dayya holds a PhD and an MPH from the University of Connecticut, grounding his clinical expertise in rigorous research, biostatistics, and a deep understanding of population health. This is a distinction that less than 3% of all physicians hold. Throughout his career, he has served in diverse, often underserved communities – from Native American reservations in California to medically underserved areas in the Bronx borough of New York City. These experiences continue to shape his commitment to thoughtful, patient-centered and Community Oriented Primary Care.
Dr. Dayya has held numerous academic and leadership roles, including serving as Associate Medical Director of Undersea and Hyperbaric Medicine at Northwell Health and as Clinical Associate Professor at the Yale School of Medicine. His work spans clinical care, research, and teaching, with published contributions in wound care, public health, and hyperbaric medicine. What ultimately led him to establish his Membership Model Practice was a desire to practice medicine differently. As he puts it, “this model gives me the ability to spend more time, go deeper, and truly personalize care for each patient.” By stepping outside traditional constraints, he has created a practice that enables more meaningful and deeper relationships, greater attention to prevention, and a more comprehensive approach to the health needs of his patients.

At the core of Dr. Dayya’s approach is a focus on quality, longevity, lifestyle, and whole-person (holistic) care. He believes that “health isn’t just about treating illness, it’s about optimizing how you live every day,” integrating preventive, functional, and evidence-based strategies into each patient’s care plan. That philosophy extends beyond the exam room. An avid paddleboarder for more than a decade, Dr. Dayya embraces movement and the outdoors as essential parts of long-term health. He has even competed in large endurance events, reflecting his belief that fitness should be both purposeful and enjoyable. He enjoys these many activities with his family. “As a physician, you have to practice what you preach,” an ethos that defines both his personal life and the elevated, highly personalized experience he provides to his patients.
